Insurance
We accept most insurance plans. We also accept payment from most PPO and indemnity plans. Please call us to make an appointment and we will answer any questions you may have.
Our office must be notified of any cancellations a minimum of 48 hours prior to the scheduled appointment. Failure to adhere to this policy will result in a $99 fee per scheduled hour. All cancellation fees must be paid prior to any future visits.
